Social Services | Pastoral Care Tallwoods Care Center provides the services of a certified social worker to meet the psychosocial and emotional needs of our residents. Whether setting up services for discharge following a rehabilitative stay or helping someone adjust to new living arrangements, the role of the social worker is to help and guide the resident and family. The role of the social worker is quite diversified. The social worker will plan, with assistance and guidance from the family, comprehensive discharge services to facilitate the resident's reentry into the community. They will also guide the resident and family in obtaining supplemental services. In addition, they can advise residents and families regarding insurance, Medicare and Medicaid issues. Should a resident require hospice care while in the facility or in the home upon discharge, the social worker will guide the family and help secure these services on behalf of the resident. We realize the importance of an individual's faith, and schedule religious services accordingly. It is a privilege of the Activities Department to assure each resident that his/her spiritual interests and needs will be met. Whether this means scheduling a Pastor, Priest or Rabbi from the individual's own faith, or utilizing lay ministers to bring religious programming to the facility on a weekly basis, the spiritual needs of our residents are a priority of the Activities Department. |
